(using VS2005)
I am seeing various problems with the button itself:
1) it shows up in HTML files on every HTML tag, of course without function
2) tooltips for functions usually halfways cover the button when hovering over it
3) the location of the button is always wrong when the source code is horizontally scrolled

the icon in HTML is:

case=1400

the overlapping tooltips is:

case=1401

you may find the refactoring sub-menu in the VAssistX menu helps, or that CTRL-right click, both of which help to work around this.

the icon being in the wrong place when you scroll sideways is:

case=1402

Cases 1400 and 1401 are fixed in build 1531.
