Are you using Windows 7? If so, are you launching the second instance of VS2008 by holding down the Shift key and clicking on the IDE shortcut on the task bar?
In VS2005 and VS2008 holding down the Shift key while loading the IDE stops addin's from being loaded. This is something the IDE does and we have no control over this.
If this is what is happening, one work around is to middle click on the taskbar shortcut instead. This will still load a second instance of VS2008, but VA should be enabled now.
