I have read the instructions in http://docs.wholetomato.com?W328, but it seems, that

HKEY_LOCAL_MACHINE\\SOFTWARE\\Wow6432Node\\Microsoft\\VisualStudio\\12.0\\Languages\\File Extensions\\.cpp

doesn't exist anymore in Visual Studio 2013. Any idea?

The FAQ entry has now been updated, thank you for pointing out the difference, I had not realised this had changed in VS2013.

You can use the following dialog to add the file extension:

Tools -> Options... -> Text Editor -> File Extension

You may need to close and reopen the files.
We will update the documentation.
