I installed Visual Studio 2008 then reinstalled VAX 1540 but the VAX is not available in the IDE. Do old versions work with new Visual Studio?

Pomidor any version of VA that was released before the VS2008 was released will not support VS2008. This general rule holds for all IDE's, for obvious reasons.

mwb1100 is quite right, you need VA 1619 or above if you are using VS2008 RTM. VS2008 beta 2 requires a slightly earlier build of VA.

Build 1619 was the first build to support VS2008 RTM.
