I have been using Visual Assist for only a week and am already wondering how I could've gone without it! :)

I have a question about the Alt+O feature. I have a web application that consists of pages, P, with files P.aspx, P.aspx.cs, and P.aspx.designer.cs. Whenever I press Alt+O in any of the files, it prompts me for which of the other two files I want to go to. However, I never want to go to the aspx.designer.cs file -- I just want it to go to the other of .aspx and .aspx.cs. Is there any way to set up which extensions are included in a given extension's Alt+O list?

Not currently. Interestingly we had a different user requesting the ability to get into the .designer.cs files via alt-o. You may find this IDE macro helpful:

since you could use it to construct a replacement alt-o command that ignored .designer.cs files.
