Hi,
under some circumstances (i can't clearly tell at the moment when and when not), if you use Refactor > Add Include, the file name in the include clause get's changed to lower case. The file name is in the correct auto completion tool tip.

We found a scenario when this would occur. It has been fixed for the next build. case=24588 |

I have had the same issue (I couldn't figure out how to reproduce it too)
Try to clean up cache and rebuild Symbol databases (under VaX->Options->Performance). Once I did it, the problem went away.
