When you use VS.Net, VS.Net 2003 or VS 2005, you can do this easily yourself. Right-click on a toolbar, select the last option: "Customize...". You'll see a new window. Drag the Visual Assist menu over the "Tools" menu, and move a little bit down (you'll see a bar where the menu will be added). This works for all the menu's.
And if you want to have a bar between the new Visual Assist entry in the Tools menu, and the next entry, drag this next entry a little bit down. Then you'll get a seperation bar.
