Using source control (I use SourceGear Vault, but I guess it's the same with VSS): Start to edit a checked-in file. MRU immediately lists the file as modified. So even if you reject the checkout, it's still listed as modified.

Another one: if you press CTRL-Z in an unmodified file, it appears in the MRU, even though no undoing is done, and even though CTRL-Z is greyed out in the Edit menu.
