After disabling/unloading VAX, restarted VC6, opened a project, enabled/loaded VAX addin. VAX loaded without errors. The VA View did not appear. After clicking on the "Class View" tab the whole IDE just disappeared.
Repro 100%.

We can reproduce the problem.

For now, restart your IDE immediately after any load or unload of Visual Assist X.

Not a problem, after all one does not unload/load VAX all the time. Just when we need to know if VAX is causing a problem or not
