VS2003.NET using standard STL - STPport is not even in the include paths. Everytime I hover over the function I get either the VS default tooltip or the VAX tooltip. No pattern.

This reminds me of bug 1288 — although I can't reproduce the two tooltips in that bug right now. |

Topic 1288 was addressed a while ago -- hovering tooltips did not respect the "Prefer default Intellisense" setting. It does now.
As for which tooltip appears... it first depends on your setting of the "Prefer" option. If checked, VA X gives MS a chance to display. MS displays, or not displays, depending on many factors -- including its knowledge of the symbol and how you moved the mouse to hover. If MS does not display, VA X tries.
If your "Prefer" option is not set, VA X tries a tooltip. If VA X does not know the current symbol, it lets MS try. |

In this case, both VAX and VS know the symbol and which one is shown is totally random. I can hover of this symbol 20 times and get different tooltips all the time. I'm not talking about the first time tooltip. |

We found a bug in 1237 that makes MS tooltips appear even if "Prefer" is disabled. This will be fixed for US English IDEs in 1238.
