I would like to make a simplified suggestion: Let the user provide a list of defines that they dont want to see, want to be grayed out, or pherhaps putting '+' next to the first line and collapsing all lines down to the #endif.
If you are using VS .NET 2003, you can use outlining to collapse the definitions, just as you suggest. If you are stuck on VS 6, sorry. This is one reason for upgrading (but let's not go off that deep end )
