How do I activate processing/syntax highlighting inside #if #if defined regions ?

This is a Visual Studio feature and you can turn this off via the following option:
IDE tools menu -> Options -> Text Editor -> C/C++ -> Formatting -> Colorize inactive code blocks in a different color
or
in VS2010:
IDE tools menu -> Options -> Text Editor -> C/C++ -> Formatting -> Disable Inactive Code Opacity
