When I have text selected and then press SHIFT+3 to insert a # I get the "Make VA Snippet Substitutions" dialog instead. Once I deselect the text than I can type a # fine. This is on VS2010 with 1833, usually across a remote desktop session if that matters.

I'm not sure if this is a bug or by design but it's highly annoying. It hits me all the time when I try to replace some text with an #include, #ifdef, etc.

Is there any way to turn this off or reassign the shortcut?

That's the "Surround selection" feature. You can it off at:
VAssistX | Visual Assist X Options | Advanced | General | Surround Selection
