One possible reason can be that you don't have a %TEMP% directory specified, where the installer is trying to extract the files to. What happens if you
- type %TEMP% to Windows's search box of the start menu and press enter? (in case of Win7/Vista)
- type %TEMP% to Windows's start -> Run editbox and press enter? (in case of WinXP)?

Does a new Windows appears with the temporary directory?
If yes, can you please find any VSIXInstaller_*.log files in that directory and send them in with the following form? They are error logs, produced by the installer.
