I have the vsip (7.1) + extras installed. When I install va-x it seems
to go ok but when I click on 'visual assist options' (toolbar or menu entry) all I get is the text
'VSIP: Developer edition, all third-party packages allowed to load'
in the output window.

I've tried installing with ENVSDK_REGKEY unset but this didn't help.

Before we do anything else, can you check the following thread(s). In particular, ENVSDK_ROOTSUFFIX

Do you have any other add-ins installed? If so, can you unload them temporarily. See if there is a conflict. Use Tools|Add-in Manager.

Do you have any other VSIP packages installed?

Righto - I had workspace whiz and something called 'managed add-ins' - which I was fiddling with to get a command prompt inside .net
I removed both and reinstalled va.net/va-x and it's working now. Then I reinstalled wwhiz (probably don't need that now I have vassist) and it still works. I can't find the install for the managed add-ins, so I can't say for sure if it was that. I reinstalled vsip and vsip extra and everything still works. Sorry I didn't just uninstall everything first. When I find the managed-addin thing I'll install it and see if it causes the problem
