I updated to this verison of VAX:

VA_X.dll file version 10.1.1301.0
VAOpsWin.dll version 1.2.0.4
DevEnv.exe version 7.10.3077.0
msenv.dll version 7.10.3077.0
Font: Courier New 13(Pixels)
Comctl32.dll version 5.82.2900.2180
WindowsNT 5.1 Build 2600 Service Pack 2
2 processors

After updating, all my key bindings and customised menus had reverted to the defaults. I can understand VAX setting bindings for the keys it uses, but I'm mystified as to why it needs to screw up totally unrelated areas of my setup.

That's not a bug, that's a "feature" of Microsoft's VSIP, whch is needed when you want to create Menus/Toolbar-Buttons for an AddIn.

See here, here, here and finally here.

Notice the Link to the interesting Statement from a Microsoft VS-Team Developer in this Thread.

While it may not be your responsibility (or within your power) to fix the bug, your installer could perhaps warn that it's going to completely trash the user's custom settings...

If I'd known that it was going to do that I wouldn't have bothered updating - it wasn't really important enough to justify the time wasted reconfiguring everything.

It does, or at least it's always asked me every time I've upgraded. The only time it doesn't ask is the initial install.
