Sometimes the 'add include' item is missing from the VAX refactor menu.
To reproduce:
- find a symbol for which an include is missing
- double click it, so the symbol becomes selected
- hover over your selection until the drop down icon appears
=> in the drop down menu, the 'add include' item will be missing

Note that if you do not select the symbol, but simply hover over it, then everything is fine.

Tested with VAX 1840, VS2010, Windows 7 x64

It was changed intentionally due to problems when the selection is not a symbol or is many symbols.
